Improving Patient Safety with PSIRF

BRIEF
This month we
introduced the new
Patient Safety Incident
Response Framework
(PSIRF) at STSFT.
PSIRF is a national framework that will help us improve
our approach to responding to patient safety incidents.
PSIRF is all about understanding how and why a patient
safety incident/event has happened. Free from blame
and judgement. This helps us to learn and improve to
make care safer for patients.
PSIRF focusses on four key areas:
Why are we doing this?
In the past, a lot of time has been spent investigating
incidents that caused 'serious' harm. We know we
can learn just as much from no harm and near miss
incidents. PSIRF allows us to focus on areas where we
know there is lots we can improve.

We have a developed a range of resources to support
you with this change:
We've created a simple, staff handbook. It gives you
lots of information about PSIRF and how the changes
will affect you.
We have also produced a leaflet for patients.

Nursing staffing update
Recruitment pipeline
There are currently 75 Nurses and
19 Health Care Assistants awaiting
pre-employment checks and/or a
start date. This includes 16 student
nurses who will graduate between
November 2023 and April 2024. There
is one return to practice nurse 34
registered nurses and we welcome
24 international nurses, 18 who
were recruited via NHSP international
and 6 who were recruited locally.
3 month nursing pipeline and projected start

International Nurses)
